The survey form is the most flexible of the survey types that are available from FORMWIZE and include all the available question types and many features required to build flexible surveys to meet your requirements. Multiple question forms may be built, which can be displayed on a single pages, or split over multiple pages.
A full reporting package is included, allowing each question to be graphed, using a variety of chart types, and styles. A full report may be built up online and saved in PDF format for distribution. Alternatively, data obtained from the survey, may be downloaded to use in other charting software.

This is an example of a simple Website Visitor Survey that is often used to gain an understanding of the demographics of visitors to your website.
In this example, the survey form is presented in a popup window, which will close following completion of the form. The real time report is built using the FORMWIZE Reporting tool, and in this example is using Macromedia Flash format graphs. |

FORMWIZE Events is a forms module which enables the online management of Events. This module allows event invitation forms to be built, and the replies saved as an attendees/non-attendees list. Unlike a standard survey form, the lists may be edited online by the events co-ordinator. Other features include standard events email confirmation, automatic closure on event full, and customised events reports.

This is an example of a simple Event Booking Form. Each Event form has two control questions which must be included when the form is built. One is to agree attendance, and the other is to collect attendee details.

FORMWIZE can easily be used to build forms which can be used to obtain competition entries such as the example here.
The difference between a competition form and a normal form is that the reporting options in a competition form can be used to select winning entries only, and futrthermore, it can choose a random number of selections from the winning entries.

This is an example of a FORMWIZE Competition form. It is constructed in the same way as a normal survey form, but the question types offered are limited to those normally used for competitions. |

FORMWIZE can easily be used to build forms which can be used for tests and quizzes such as the example here.
A number of options exist to set the pass mark for the quiz, show the total score after the quiz, display the correct answer following submission of each answer, or all at the end, and options
to divert to alternate URL's depending upon the result of the test.

This is an example of a FORMWIZE Test form used as a General Knowledge Quiz. The questions are displayed one at a time, and the answers given after each question.
The pass mark is set at 90% and a pass message or fail message is displayed depending upon the result. |
